Superpower: Weakness Pool Manifestation
The ability to create a manifestation of all your enemy's weaknesses which you can attack to deal damage.
When activated, every being or object will appear to you with a swirling pool of energy near them. This pool is a representation of all their weaknesses, even the unsuspected and less obvious ones, like a need for food or air, an attachment to a loved one, or a past trauma. Only you can see it and only your attacks can affect it. Any damage dealt to the pool will be reflected on their owner and any type of attack will work. Basically, that pool now becomes their most obvious weakness to target.
The good:
Even attacks which shouldn't affect them will work. You can strike the pool of an intangible being.
Even those who claim to be invincible can have a weakness, not just a magical or a physical one.
Only you can see the pool. So, it will be difficult for your enemy to defend it without seeing it.
The bad:
The weakness pool can be difficult to target.
One with very few weaknesses will have a very small pool. One without weakness won't even have one.
One who is changing or shifting their weakness will either have a fast moving pool or a fading one.

Illusions wouldn't have pools, unless you meant creating illusions of swirling pools. Basically, anything that can decrease their chances to hit the pool can work, from running around to bad luck. Anti-telepathy will certainly work by preventing the pool from appearing. I don't know how Immutability would interact with this one though.

Just use an illusion power or something like distance locker to prevent them from hitting it, or just blind them. It doesn't matter if they can't hit it.

How would you defend yourself against such attacks? Is Immutability(no change to your body) or anti-telepathy (they can't read your weaknesses) enough or do you need to have this power to be immune?
